Serves: Legal Custodians & families w/ no legal designation in Monroe & Wayne counties

Services:

• Meeting in home to provide emotional support
• Provide referrals & connecting to services/resources (contact Laura Weaver for more info on group)
• Offers monthly support & educational groups as well as family and caregiver events
• Help applying for eligible financial assistance
• Connection to mental health services

Caregivers must enroll into the program – which is free of charge – to receive services
