Address: 1099 Jay Street Rochester, NY 14611 Eligibility: Serves non-parent caregivers of children with no formal legal designation, legal custody, legal guardianship, and post-adoptive families, residing in Monroe, Schuyler, Steuben, or Wayne county. Catholic Charities Family and Community Services (CCFCS) provides supportive services to Post-Adoptive, Legal Guardianship, Kinship and KinGAP families. These services are designed to help support youth permanency in the family home by offering parents and caregivers needed resources, advocacy, and caring peer connections. Participating families will be connected to a Program Case Manager who will provide support services related to: The Program’s Case Managers will also assist families with referrals to other community resources as needed.
