Virtual Case Management

Virtual Case Management:

Virtual Case management is personalized assistance provided by the Kinship Navigator in certain counties where there is no local Office of Children and Family Services (OCFS) funded kinship program that serves all caregivers. It is a pilot project developed in conjunction with OCFS and funded by the federal Children’s Bureau.

The counties are:  Fulton,  Livingston, Orleans, Otsego, Rensselaer, Saratoga, Schuyler,  Warren, Washington, and Wyoming.  If you live in one of these counties, you qualify for services.

After initially speaking to a kinship navigator, you’ll be assigned a Regional Coordinator who will provide virtual case management. The Regional Coordinator has in-depth information about kinship law and resources and will continue to be available to you after the initial call.

The Regional Coordinator Can Help By:

-Identifying what needs you and your family have
-Building a case plan to help meet your needs
-Assisting in applying for public benefits, including financial assistance and Medicaid
-Making assisted referrals to local agencies that offer services in your community
-Conducting follow up calls to ensure your needs have been met
-Offering ongoing phone support for any questions or concerns you may have.
