The Leadership Training Institute’s (LTI) Kinship Caregiver Program provides case management and supporting services for anyone who is raising someone else’s child.  The target populations are children and youth residing with their relative caregivers, regardless of whether the children are in Kinship Guardianship or Kinship Foster Care, are placed directly by the Court in a relative’s care, or are among the larger cohort of children residing informally with kin. This includes children placed by the court under Article 10 with LDSS supervision, or court-placed youth under Article 6 with no LDSS involvement. The target population can include kin such as grandparents, aunts, uncles and cousins as well as non-related “fictive” kin with a significant relationship to the child if they are caring for the child.LTI in Hempstead will provide and/or coordinate the following core components directly or through referral in partnership with other stakeholders including the Nassau and Suffolk counties Department of Social Services:

* Component I consists of providing case management services for kinship families.

* Component II of this model is for supportive services for kinship families including information, referral and advocacy:

  1. Trauma screenings
  2. Trauma informed services
  3. Outreach to promote community awareness and early family engagement
  4. Information, referral, advocacy and linkage to community resources
  5. Early, comprehensive and on-going family assessment
  6. Coordination/collaboration with other service systems to address family-specific needs
  7. Parenting training/information
  8. Crisis intervention services
  9. Home visits
  10. Support Groups

The Suffolk County Office for the Aging is the designated Area Agency on Aging under the Older Americans Act. For over 40 years, Suffolk County Office for the Aging has administered federal, state and county programs for persons 60 years of age and older.

Specific goals for the Office are to develop a comprehensive service system designed to assist persons 60 years of age and older in maintaining maximum independence and dignity, to remove barriers to economic and personal independence, and to provide a continuum of care for the vulnerable elderly. Most importantly, Suffolk County Office for the Aging acts as advocate on behalf of all Suffolk County senior citizens.

The Huntington Youth Court Program is run for youth by youth. Youth offenders under 16, who have committed their first misdemeanor crime, are given the opportunity to have their case heard by a jury of their peers. Volunteer youth perform the roles of prosecuting and defending attorney, judge, bailiff, and jury.

The Huntington Drug & Alcohol Counseling / Education Center is a state licensed, outpatient chemical dependency treatment prevention provider. We are a community based, not-for-profit agency licensed and funded by the New York State Office of Alcohol and Substance Abuse Services, in conjunction with Suffolk County and the Town of Huntington. We offer individual, group, couples, and family counseling to individuals who are abusing substances, those who are at risk of becoming drug involved, and their families. Prevention Services include presentations in schools and throughout the community on various health-related topics.

REACH Community and Youth Agency, Inc. is a community-centered, non-profit organization providing social, recreational, informational, educational, and advocacy programs for children, youth and families of Commack and Half Hollow Hills.
Since 1972, REACH CYA has identified the concerns and needs of youth, parents and schools, and offered activities that develop positive attitudes, self-esteem and problem solving skills.
Over 1,300 youth participate in CYA programs yearly. Programs are held in the schools, Manasquan building in Half Hollow Hills and Teen Recreation Center at Old Farms School in Commack. In addition to our two sites, Youth Workers facilitate programs and outreach in schools and other popular teen locations throughout the community.
REACH CYA also administers before and after school child care through our Children’s Center. The Children’s Center is a New York State licensed child care program for elementary school age children in Half Hollow Hills. The Children’s Center also offers a half day Preschool Program for 4-year olds in Half Hollow Hills.
